Association has an exciting new solution for
your dental insurance and eye care needs.
ORTA has partnered with Association Member
Benefits Advisors (AMBA) to offer a choice
of two dental insurance plans and a new eye
care plan with progressive lens coverage!
You have the option of choosing a dental
insurance plan which features very
affordable premiums, or a plan which offers
significantly richer benefits – it’s your
choice! These new plans have been heavily
negotiated for ORTA and provide exceptional
benefits. The
only requirement to participate in these
plans is your continued membership with ORTA.
The Platinum and Gold dental insurance plans
are available for you to choose from. Below
are some of the outstanding features of the
Platinum dental insurance plan.

With dental costs increasing rapidly each
year, we believe that you will find the
comprehensive benefits of the Platinum plan
a welcome addition to your overall health
care package. For more budget conscious
members, we offer the Gold dental insurance
plan with lower premiums and higher
co-payments.
The eye care
plan, through EyeMed, not only covers
progressive lenses, but it provides you with
access to EyeMed’s vast, nationwide network
of ophthalmologists and optometrists. You
may also choose from national leading
optical providers such as LensCrafters®,
Sears Optical®, Target Optical®, and most
Pearle Vision® locations. When visiting a
network doctor you will receive an eye exam
and lenses once every 12 months and frames
provided once every 24 months after a
minimal co-pay.
Without EyeMed, you can expect to pay an
average of $87 for an eye exam, $216 for
single vision glasses, $253 for bifocal
glasses, and $327 for progressive lenses
glasses. However, with EyeMed your eye exam
is fully covered after a $15 co-pay, and
your prescription glasses are fully covered
up to your allowances after a $25 co-pay.
